Anyone interested in the wrestling team and attending any summer conditioning or practices when school returns in August needs to complete their sports physical, which I have a link for you to download the physical form to print if needed on the home page. After you have your physical completed then you need to complete all Final Forms online. Click the link provided below and a parent will need to complete the parent portion before the student/athlete can complete their portion of the forms.
